Do you mean what does the form represent? If so, it is, Kokopelli, a Native American diety.

I don't know the maker marks, though.

This is a wall decoration sold at Lowe's Home Improvement Stores about 7-10 years ago. It is supposed to be a musician of some type. I know this to be correct because I bought one myself.
